Swelling or puffiness around the eyes is a common concern–one that can affect people of all ages and make you look tired, stressed, or unwell. In some cases, puffiness is temporary. But for many, it becomes a persistent cosmetic or even functional issue.
At her specialty practice in Irvine, Dr. Melanie Ho Erb offers expert evaluation and treatment for puffy or swollen eyelids, helping patients across Orange County and Los Angeles address both the root cause and the appearance.
Whether your swelling is due to aging, allergies, fat herniation, or previous fillers, Dr. Erb provides precise, customized solutions to restore a smooth and refreshed eyelid contour.
After doing research on blepharoplasty and reading reviews on different Oculoplastic surgeons I decided to visit Dr. Erb. I had an issue with under eye bags and I tried every product under the sun and nothing helped to reduce my bags/puffiness. After my visit with Dr. Erb she put me at ease and gave me her suggestion. So on New Year’s Eve I decided to have my surgery! I was extremely nervous to get the surgery because I’ve never had any type of surgery done on me before. Her staff did a great job to walk me through everything I needed to do before and after surgery. After I was done with my surgery I had a little bit of pain but when I got home took a Tylenol and that was the only time I had to take something for pain. I followed her instructions to the “T” and I didn’t have any issues. I’m very happy with my results and highly recommend Dr Erb!

Common Causes of Puffy Eyelids
Puffiness can affect the upper or lower eyelids and may stem from a variety of underlying factors, including:
- Orbital fat herniation, where fat pockets push forward with age
- Fluid retention or lymphatic congestion, especially in the mornings
- Genetics, particularly with under-eye bags or early fat prominence
- Allergies or chronic irritation that cause swelling and inflammation
- Dermatochalasis, or loose skin that folds over fat pads
- Complications from prior cosmetic treatments, such as fillers placed too superficially
Dr. Erb’s detailed facial and eyelid exam helps distinguish between true puffiness from fat pads, temporary swelling, or volume imbalance that may be treated non-surgically.
Eyelid Surgery and Non-Surgical Solutions for Puffiness
When puffiness persists despite home remedies or skincare, surgical options may offer long-lasting relief. Depending on your anatomy, Dr. Erb may recommend:
- Upper eyelid blepharoplasty to remove fat or excess skin
- Lower eyelid blepharoplasty (transconjunctival or external) to reposition or remove herniated fat
- Filler reversal or correction if previous injections contributed to swelling
- Laser skin tightening or resurfacing for fine lines and tissue support
- Injectables to balance under-eye hollows or create smoother transitions
Each treatment is tailored to the underlying cause and your personal aesthetic goals.
