Please bring these items with you on your initial visit:
- Completed intake forms either on paper or in the office by arriving 10 minutes before your appointment time. We emailed these forms to you.
- Use your phone to take pictures of old photos of how you looked 20 years ago so that Dr. Erb gets an idea of what to aim for.
- If you have had fillers placed in your lower lids/tear hollows, bring photos of how you looked just before the filler was placed.
- Your list of questions for Dr Erb.

- To get to our office suite, we are located on the 10th floor. You take 2 separate elevators. Take the 1st elevator to 9; walk down the hall; take the 2nd elevator to 10. We are in Suite 1007.
- If you want injections of neurotoxin or fillers on your first consultation visit, then do not take aspirin, ibuprofen, fish oil, flaxseed oil, or turmeric for 1 week before your visit.
